Die saure Phosphatase im Serum besteht aus 5 Isoenzymen. Sie stammen vor allem aus den Erythrozyten, Thrombozyten, Zellen des retikuloendothelialen Systems in Milz und Leber, aus der Niere, aus Knochen und aus Prostata-Epithelien. Das Isoenzym 2, die Prostata- spezifische saure Phosphatase, wird überwiegend, aber nicht ausschließlich, in der Prostata gebildet.

Bei progredientem und metastasierendem Prostatakarzinom ist meist ein Anstieg sowohl der gesamten sauren Phosphatase als auch der Prostata- spezifischen sauren Phosphatase zu finden. Die Erhöhungen sind bei 80 % der Kranken mit metastasierendem Prostatakarzinom abhängig vom Stadium. Die prozentuale Erhöhung in jedem Stadium hängt von der Klassifizierung ab (pathologisch oder klinisch).
